Health Department creates the Integrated Care and Social-Health Coordination Service

The Department of Health of the Generalitat Valenciana has approved Order 3/2026, of September 9, which incorporates the new Integrated Care and Sociosanitary Coordination Service into the structure of the Regional Secretariat of Health, configured as the administrative unit responsible for the functional coordination of sociosanitary care, chronicity, fragility, palliative care, and long-term care. The regulation, signed by the regional minister Marciano Gómez Gómez, is published in the Official Gazette of the Generalitat Valenciana number 10449, of September 11, 2026, and enters into force the day following its publication.

The order modifies Order 3/2025, of April 4, which develops Decree 22/2025, of February 11, of the Consell, approving the organic and functional Regulations of the Department of Health. Through this, the Valencian model of integrated care is institutionally consolidated, following the trajectory initiated by the Regional Office for Sociosanitary Coordination and Integration, created by the regional minister’s resolution of October 3, 2023, which maintains its nature as a body for strategic impulse and coordination.

A new regulatory framework

The measure is framed within the recent Law 3/2026, of June 29, of the Generalitat, on urgent measures against hyper-regulation, the streamlining of procedures, and the guarantee of market unity. Its article 92 modifies article 46 of Law 10/2014, of December 29, on Health of the Comunitat Valenciana, reinforcing the unity and integration of health and social information.

• In collaboration with the Valencian Public Social Services System, an information integration system is planned to create an electronic sociosanitary space for the secure exchange of data between health and social professionals.
• The first additional provision of Law 3/2026 promotes coordination and technological support between the departments of health and social services for disability assessment, using standardized medical and psychological report models.
• The fourth additional provision enables the communication of data between the Valencian Health System and the Valencian Public Social Services System without the consent of the affected persons, incorporating security, traceability, and encryption measures.

Functions of the new service

The Integrated Care and Sociosanitary Coordination Service assumes a wide catalog of functions, among which the following stand out:

• Coordinate health and social health services and propose the adaptation of care pathways.
• Promote functional integration between the health and social systems and person-centered continuity of care.
• Develop strategies for the detection of complex chronic patients, people in situations of fragility, and patients with palliative needs.
• Coordinate integrated care models for people with long-term care needs.
• Promote the development of the electronic social health space and collaborate on standardized report models for disability assessment.
• Provide technical and administrative support to the Social Health Coordination and Integration Office.

At no additional cost

The order modifies articles 4, 5, 6, and 8 of Order 3/2025, reorganizing the administrative units dependent on the Autonomous Secretariat of Health and the general directorates of Primary Care and Hospital Care. According to its sole additional provision, the application of the regulation will not entail an increase in public spending and will be managed with existing personal and material resources.

Entry into force occurs the day following its publication in the official bulletin, and any provisions of equal or lower rank that oppose the order are repealed. With this reform, the Generalitat reinforces the operational capacity of the health Administration to respond to the needs derived from aging, chronicity, and dependency, guaranteeing comprehensive and person-centered care.
